For healthcare professionals

Report a side effect.

If you are taking part in one of our studies, tell your study team straight away. They report it for you. For any other medicine, tell your doctor, pharmacist or nurse, or use the MHRA Yellow Card scheme, which anyone can use.

If you feel unwell now, call NHS 111, or 999 in an emergency.

Tell our team

Tell us about it.

This is a design concept. Nothing you type here is sent or stored. To report a real side effect, use the Yellow Card scheme or tell your doctor.

Who are you reporting for?

In your own words. Include anything that made it better or worse.

When did it start?

For example, 27 3 2026. A rough date is fine.

The study code is on your card, for example AFR-0311-101.

On a real site, our safety team would use this only if they need to ask more.
